Over the past twenty years, hockey‑related ocular trauma has accounted for an estimated 6,466 injuries across the United States, according to a recent study published in JAMA Ophthalmology. While the overall injury rate fell between 2004 and 2018, the data show a modest resurgence in the most recent five‑year period, suggesting that progress may be stalling.
The investigation found that 83 % of these injuries were directly linked to equipment — sticks, pucks or balls — rather than to contact with other players. Minor anterior segment injuries comprised the majority of diagnoses, occurring in 63 % of cases, yet 12 % of the documented ocular findings were classified as vision‑threatening. Notably, adults sustained every major posterior segment injury identified, while pediatric patients were far more likely to experience the less severe anterior injuries.
Demographic patterns emerge clearly: 79 % of injured players were male and 69 % were under 18 years of age. Among male patients, 21 % suffered major anterior segment injuries, whereas only 2 % of females experienced such injuries. Despite the high proportion of equipment‑related trauma, protective goggles or glasses were recorded as the cause of injury in only four patients, underscoring a striking gap in the use of preventive gear.
Researchers stress that prompt, thorough assessment of any ocular trauma is essential to minimize the risk of permanent vision loss. The low rate of documented protective eyewear use raises concerns, especially given the established protective benefits of visors and face shields. Public‑health initiatives that combine education with consistent enforcement of eye‑protection policies could help reverse the recent upward trend and safeguard the sight of thousands of players.
